Double-Eyelid Surgery
“Double-eyelid Surgery” or supratarsal crease creation is a very popular procedure among the Asian population. Asian eyelids are structurally different from Caucasian eyelids in several different ways:

  1. Lack a supratarsal crease (double eyelid)
  2. Thicker and fattier eyelids
  3. Hooding of the eyelid skin in the middle corner of the eye (epicanthal fold)

Each of these anatomical differences vary in their presentation, ranging from a fully “westernized” eye that has a full supratarsal crease and no epicanthal fold, to one that lacks a supratarsal crease, has thick swollen lids, and medial skin hooding.

Some people have supratarsal creases all the time. Some people have creases that are only evident when they’re tired. Supratarsal creases also vary in their height from barely being visible to those comparable to Caucasian eyelids. 

Similarly, the medial eyelid skin hooding varies from barely being there to a dramatic prominent redundant hoot that gives the eye a slanted and pulled down appearance.

Asian eyelid surgery can correct all these anatomical differences. The amount of surgery involved depends on what the patient finds undersirable and their underlying anatomy. Procedures can range from a supratarsal crease creation to a full westernization of the Asian eye.

Cheek Reduction
Prominent cheeks often gives a person the appearance of being fat or simply unattractive. In asian society, having a round, moon-shaped face is considered unattractive.  Cheek reduction surgery can give your face contour by removing some unnecessary buccal fat to offset a round face and full cheek. Patients who have a history of grinding their teeth at night and sore jaws may also benefit from simultaneous botox injections to their masseter muscles. It would decrease the extra muscle bulk that resulted from constant use when your teeth grinds at night.

Jaw Reduction
The jawline defines the aesthetic contour of your lower face. Prominent jaw bones can make a face seem excessively wide and disproportionate to the rest of the person. It can also make them seem fat and add to the rounded appearance of the face. Jaw reduction surgery can narrow your cheek and make the face appear less round.

Nose Surgery
Asian noses frequently appear short, lack a prominent nasal bridge, and lack nasal tip projection. Unlike Caucasians, whose noses usually require a reduction from having a nose that is too prominent, Asians usually require a dorsal augmentation to increase both the dorsal height and tip projection.  This augmentation can be performed using either a silicone prosthesis or a patients own cartilage. While L-shaped silicone implants work well and has no associated donor site morbidity, there is always a risk of implant infection and extrusion. Autologus cartilage augmentation techniques are much less likely to get infected or extrude, but is associated with a donor site morbidity that many patients find unattractive.

Calf Reduction
Long, thin calves are considered beautiful. It not only makes a person appear taller, but also conveys a sense of grace and elegance. Calf reduction surgery was first developed in Korea to address a bane of many Asians, disproportionately prominent calves that are both short and muscular. Many procedures have been developed to make calves look less prominent. These include liposuction, botox injections, radiofrequency nerve ablation, and partial muscle resections. The right procedure for a patient depends on anatomy and appearance of the calf.
